103 Scholarships & Military BAH Only students who are receiving scholarships not given by WCU and students who receive military BAH or whose parents receive military BAH are required complete this form.
104 Student Athlete Outside Scholarship Certification All student athletes who are receiving scholarships not given by WCU are required complete this form.

601 Proof of Support for Dependent Child or Legal Dependent For students who want to change their status from dependent to independent based on the fact that they provide more than half support for a child or legal dependent.

603 Petition for Independent Status For students who have extraordinary circumstances that qualify them for review of dependency status.
604 Dependency Override Continuation for Returning Students For students who have been approved for dependency override in the past year.
606 Prior Year Homeless Youth For students who were previously determined to be classified as unaccompanied homeless youth by the financial aid office.

702 Change of Graduation Date For students who were scheduled to graduate Fall 2015, but will not graduate until Spring 2016.
703 Prerequisite Form for Program of Study For students required to take undergraduate courses required for their program of study.
704 Parent Refusal Form For students whose parents refuse to provide the information necessary to complete the parent portion of the FAFSA.
705 Consortium Agreement For students who are enrolled at another institution but taking approved coursework leading to a degree at Western Carolina University.
707 Prerequisite Form for Teacher Certification For students who are in the teacher certification program and are required to take specific courses as prerequisites to the courses for the certification.
708 Total and Permanent Disability Certification For students who have previously had student loans discharged because of total and permanent disability.
